Available Drugs

  • Performance Drug List for Clients with Advanced Control Specialty  This guide lists preferred drugs within select non-specialty therapeutic categories to help identify products that are clinically appropriate and cost-effective.
  • Specialty Pharmacy Drug List  Attached is a list of medicines that are considered Specialty Medications. This list is simply a resource to understand if a medication is considered Specialty and does not tell you if the drug is covered. For a list of covered Specialty medications please refer to the Advanced Control Specialty Formulary list.
  • Advanced Control Specialty Formulary  This guide lists preferred drugs within select specialty therapeutic classes to help identify products that are clinically appropriate and cost-effective.
  • Preventative Drug List  If you are enrolled in a high deductible health plan (HDHP), our Preventive Therapy Options program offers you savings on prescriptions that you take regularly to prevent certain health conditions. This cost-saving program allows you to pay a copay* for your medications even if you have not yet met your annual plan deductible. You won’t have to pay full price for the medication until your plan deductible is met; you will only pay a copay.
  • CVS Caremark Drug Exclusion List  Attached is a list of medicines by drug class that will no longer be covered by your plan’s drug list. If you continue using one of these drugs after 04/01/2012, you may require to pay the full cost.
  • Compound Exclusion List  Attached is a list of compound medicines by drug class that will no longer be covered by your plan’s drug list. If you continue using one of these drugs after 09/01/2014, you may be required to pay the full cost.
  • Smoking Cessations coverage. Days’ supply limit for smoking cessations is 90 day supply annually.
  • Tobacco Cessation Prescriptions  With a prescription from your doctor, you are eligible to receive no-cost tobacco cessation prescriptions or Nicotine Replacement Therapies (NRT):
    • A 12-week supply of generic nicotine replacement products patch, gum and lozenges. Including Bupropion (generic of brand Zyban) Zyban is NOT covered. Brand-name Chantix (varenicline tartate) is covered
    • Tobacco cessation examples (OTC and Rx): Bupropion HCl tablet SR 12 hr 150 mg, Nicotine TD patch 24 hr kit 21 mg, 14 mg and 7 mg/24 h, Nicotine polacrilex gum 2 mg and 4 mg, Nicotine polacrilex lozenge 2 mg and 4 mg, Chantix tablet 0.5 mg and 1 mg, Chantix tablet 0.5 mg x 11 tabs and 1 mg x 42 pack
    • No cost to you for up to 2 quit attempts each year
    • You must fill the prescription at a retail pharmacy and use the pharmacy counter for the NRT products with your CVS/caremark card
    • The deductible and coinsurance do not apply for the tobacco-cessation products listed above; Additional tobacco-cessation products may be available that are subject to regular plan coinsurance for both the CDHP and PPO
