Maintenance prescriptions (i.e., maintenance medications typically prescribed to treat long-term or chronic conditions) must be filled in 90-day supplies at CVS Pharmacy or through CVS Caremark Mail Service Pharmacy unless you actively opt-out each year by calling Customer Care.
Some covered medications could be changing. To view your options, please review the Formulary Lists under the Drug List section. If you don’t change to a covered medication, you’ll have to pay 100% of the cost for the targeted medication. These costs will also not apply toward your deductible or out-of-pocket maximum. You can ask your doctor to submit a prior authorization request; if approved the targeted medication will continue to be covered.
