CVS Caremark Drug List Information for Prescribers

The CVS/Caremark Prescribing Guide contains useful information for physicians. This Prescribing Guide is not specific for GEHA participants, however would be helpful information for physicians.

CVS Caremark Forms

  • Mail Order Request Form
  • Paper Claim Form (Direct Reimbursement Form)
  • Mail: You may submit a paper claim via the CVS Caremark Prescription Drug Reimbursement Form
  • Electronically: You may submit for reimbursement online via Caremark web portal (Caremark.com) or the Caremark mobile app (available for Androind and Apple)
  • Web: Log into your Caremark.com profile; under Plan and Benefits select Submit a Prescription Claim; completed all required fields and submit.
  • Mobile App: Open or Download the Caremark mobile app: under Plan and Benefits select Submit a Prescription Claim; completed all required fields and submit.

Over-the-counter (OTC), at-home COVID-19 tests

If you purchased an at-home COVID-19 test on or after January 15, 2022, you may be eligible for reimbursement. As long as the test is used to diagnose a COVID-19 infection for yourself or a covered family member, you can request reimbursement through your Caremark.com account.
