About your prescription drug plan
This is where you'll find information about your JPMorganChase Prescription Drug Plan as you prepare to enroll in your benefits. Learn how much you'll pay, ways you can save and where you can go for your prescription drug needs.
What’s changing for the 2025 plan?
In 2025, the Medical Plan option you choose (Option 1 or Option 2) corresponds to the Prescription Drug Plan option you’ll receive. Plan summaries and check drug cost tools are included on this web site.
Key features of the 2025 plan:
- There is no deductible - just fixed dollar co-payments for all covered drugs
- Lower generic drug copay of $5 for a month supply and lower co-pays for preferred brand drugs
- There is a separate, lower annual out-of-pocket maximum (safety net), which is the maximum amount you’ll pay for covered services across the Medical and Drug Plans
- Preventive brand and generic drugs continue to be free
Note: Co-payment (“co-pay”) and coinsurance are amounts a member is required to pay for a prescription in accordance with a Plan, which may be a deductible, a percentage of the prescription price, a fixed amount or other charge, with the balance, if any, paid by a Plan.

Check drug availability and cost
Review the formulary drug lists (specialty and non-specialty).
- 2025 CVS Caremark Standard Drug List
- 2025 CVS Specialty Drug List
- CVS Caremark Brand and Generic Preventive Drug List (Some strengths or dosage forms may not be included in the Brand and Generics Preventive Therapy Drug list and certain drugs, products, or categories may not be covered regardless of their appearance in this document. Certain drugs are subject to step therapy, prior authorization or quantity limits. Please note, the Mandatory Generic Drug Program applies. If you fill a prescription for a brand name drug when a direct generic equivalent is available (e.g., if you fill Lipitor instead of atorvastatin), you will pay the entire cost difference.)
NOTE
- Please check these drug lists frequently for up-to-date information.

Your costs
Know what you'll pay out-of-pocket for any brand name or generic equivalent prescription drug.

Save with generic drugs
The use of generic drugs is a key way to control your costs under the JPMorganChase Prescription Drug Plan. If you or your doctor requests a brand name medicine when a direct generic equivalent is available, you'll pay the difference in cost between the brand name and generic, plus the generic co-pay (i.e., the brand name drug cost minus the direct generic drug cost plus your generic co-pay).

Find a pharmacy in your area
Find pharmacies near you that accept your coverage. Pharmacies in the CVS Caremark retail network generally cost you less than non-network pharmacies.
